How the Hypertension Program Works

2. Take Readings at Home
Patients are asked to check their blood pressure at least one to two times per week. Daily readings are encouraged when possible.
3. Automatic Data Collection
Readings are securely collected through the Medsense care platform using a built-in cellular connection—no smartphone or Wi-Fi needed.
4. Receive Your Blood Pressure Reports
Patients can receive reports by email, mail, through their partner pharmacy, or via the optional Medsense mobile app. The mobile app provides immediate access to readings and trends as soon as they are recorded.
